Helpful Details for New Patients
You will need to complete a Registration and Medical History Form to help us get to know you. The doctor will use this information to create your own customized chiropractic care plan.
Contact us ahead of time and we can email you a secure personal link to an electronic form that you can complete online. Or, you can complete your intake information before your first appointment. Simply CLICK HERE, print the Registration and Medical History Form, and complete it before you arrive.
You’ll meet one of our treating chiropractic physicians who will review your health history and determine if yours’ is a chiropractic case.
Standard physical, orthopedic, neurological and chiropractic tests will be performed to determine the cause(s) of your problem.
If the doctor determines yours is a chiropractic case, they will most likely begin chiropractic treatment that same day.
While your first visit generally takes about 45 minutes, typical follow-up visits will last about 5-10 minutes.
We accept all forms of health insurance including auto accident and work related claims. As a courtesy, we can file insurance claims if provided with the necessary information. A staff member will call to verify coverage with the insurance company and then discuss payment options with you to the best of our understanding. We strive to complete this process by your second visit.
Patients are responsible for payment of those fees not covered by their insurance; including deductible and co-payment. We realize that some of our patients have little or no insurance coverage, therefore we offer a medical discount plan through ChiroHealthUSA.
We accept almost all insurance including Medicare, and are a preferred provider for most major and local health insurance networks including SummaCare and AultCare.
